Before You Begin: A Few Safety Tips

We want you to feel as safe as possible when using this form. Before you begin — or if you need to leave quickly — please read the information below.

Choose a Safe Moment

Consider filling out this form when you are alone, in a private space, and unlikely to be interrupted. If possible, use a personal device that others don’t have access to, rather than a shared computer.

What the “Leave this Page” Button Does and Doesn’t Do

The Leave this Page button is designed to help you exit this form quickly. Here is exactly what it does:

It will:

  • Redirect your browser to an unrelated page immediately.
  • Remove this form page from your browser’s session history, so that pressing the Back button will not bring someone back to it.

It will not:

  • Erase any other pages you visited before arriving at this form. If you visited other websites related to support or resources earlier in your session, those pages may still appear if someone presses the Back button or looks through your browser history.
  • Close your browser or other open tabs.
  • Erase your browsing history automatically.

For best protection, consider opening this form in a private or incognito window next time. If you are concerned about pages already visited, you can also clear your browser history manually.

Information on Confidentiality

Please note that this form is to offer a pathway to disclose information with the institution to help inform our understanding of the events impacting our community, to better plan actions, care and training. Anonymous disclosure will be received by the SVSC team, but won’t trigger immediate support or actions. Support is available on our website regardless of how much you disclose.

This form addresses experiences of sexualized violence, including sexual assault, harassment, coercion, and abuse. These topics may be distressing. Please take care of your well-being while completing it.

This space allows you to share an experience(s) anonymously. You choose what to share and what to skip. This is not a formal report and does not start an investigation.

  • This is not a report.
  • This is not a complaint.
  • You are not required to take next steps.
  • You may choose to make a formal report later, or not at all.

Definitions

  • Anonymous: You do not need to provide your name or identifying details.
  • Confidential: Information is handled carefully and only reviewed by trained staff; it is not widely shared.

Limits of confidentiality

Confidentiality may be limited where there is:

  • Immediate risk of serious harm.
  • Risk to a minor.
  • A legal obligation to act.

This space is intended to support respectful and genuine sharing of experiences or concerns related to sexualized violence.

Information may be used in a de‑identified way to:

  • Provide support.
  • Identify patterns.
  • Improve prevention and safety.

Information is stored securely in accordance with university policy and Quebec privacy law.

If you later choose to make a formal complaint under the Sexualized Violence Policy, information on how to do so is available online.

The “Leave this Page” button will redirect you to an unrelated page and remove this page from your browser history — but it will not erase other pages you visited before arriving here. We recommend reading the safety tips on this web page before filling out this form, and to choose a safe time and place. You can also learn how to clear your full browser history after your session.
