Individuals born in Quebec must submit:
- copy of birth certificate
Individuals born in Canada outside of Quebec must submit:
- copy of birth certificate
AND
- copy of Quebec health care card
AND EITHER
- a signed affidavit attesting you have not been studying full-time in the past twelve months.
OR
- a copy of your lease plus two different documents attesting to receipt of mail by you at the address listed on the lease
Individuals born outside of Canada must submit all of the following:
- copy of Quebec health care card
- copy of both sides of permanent resident card
- copy of CSQ (Certificat de sélection du Québec)
